For this exercise you will need to open the ICD-10-CM Index to Diseases and Injuries, ICD-10-CM Index to External Causes, the ICD-10-CM Table of Drugs and Chemicals, the ICD-10-CM Tabular List, ICD-10-CM Official Guidelines for Coding and Reporting, and Coding Clinic for ICD-10-CM and ICD-10-PCS. The purpose of this exercise is to demonstrate that you can follow the basic coding steps.

1. Diagnosis: Acute lacunar infarction and encephalopathy due to infarction. While in a patient room in the emergency department, tissue plasminogen activator (tPA) was administered. This tPA administration resulted in cerebral hemorrhage as an adverse effect of tPA, initial encounter.
