Every time a measurement deviates from the target value, the worker adjusts the machine toward the target value. Why does this conscientious approach result in poor quality?

It means that machine has surely some draw backs in terms of improper setup/calibration or outdated technology or any reason and so every time it needs to be adjusted. Parallely time is wasted. If any machine has certainly some defects then this conscientious approach also gives result of poor quality.
