If the family of a patient in the intensive care unit submits the individual’s living will document, should it be honored immediately or should there be a discussion between physicians and the family? Who should make the decision? Why?

I think there should be a discussion between physicians and the family because yes the patient is in intensive care unit and all the health conditions possible are not good , but still family should discuss all the rarest possible chances for the survival of pateint and not loose hope. I thinks as per the health's conditions i think the decision should be made by the physician that whether the individual's living will should be discussed at this moment or not as they more properly know the conditions of the patient. Health status also plays a very important role in making such decisions. It could happen that due to any kind of differences or benefits, the family is more willing to honor the will as soon as possible, but as the physician will never do something that is out of their job's oath and will help in giving the best possible advice , so i think that there should the possible discussion between them as well and the decision should be made by the physician. But not necessarily that everyone should think in the same manner as i did.
