  • 2,3-biphosphoglycerate (BPG) promotes hemoglobin transition from a high oxygen affinity state to a low oxygen affinity state.
  • During storage of blood, RBCs rapidly lose 2,3-biphosphoglycerate leading to an increase in the affinity for oxygen and a temporary impairement of oxygen transport.
  • Decreased survival and organ function have been demonstrated with transfusion of low 2,3-DPG red cells.
