Predict the Diagnosis!

Patient SB is a fifteen year old high school freshman. SB noticed the transition from middle school to his college preparatory high school to be quite difficult, and as a result, his motivation to complete his course work has reduced greatly. Thinking he just has a case of the “blues” because he is not adjusting like his peers, SB decides to go through the motions of high school until his mood improves. Unfortunately for SB, his lack of motivation to complete his course work continues and he begins noticing an inability to enjoy his favorite video game, “Call of Duty” because the game doesn’t seem to make sense to him anymore. For example, while playing “Call of Duty,” SB notices that he cannot remember where the enemies are hidden, or even concentrate enough to shoot at them as they appear on the screen. SB also hears the other online players calling to him by his real first name, and not his screen name (he has never provided his real name). Growing concerned, SB decides to talk to his parents about his recent observations. His parents, also growing concerned, have also noticed that SB hasn’t spent time with his friends in over three months, and hasn’t played guitar, which he typically finds very pleasurable. His parents have also noticed that SB is even more difficult to communicate with, given that his sentences are typically disjointed and incoherent.

a. What symptoms is patient SB presenting?

b. Given the symptoms listed in part a., which disorder is SB most likely suffering from?

c. SB was adopted at a very early age. His rearing parents, who have provided a very stable home life for SB, decided to investigate his biological family history. What biological and environmental factors may have put SB at a higher risk for this disorder?
