You are the director of health information services at a tertiary-care hospital. You and the director of emergency room services are jointly responsible for reporting instances of communicable disease, child abuse, and cancer to the appropriate state authority. You have just completed an audit of your institution’s reporting mechanism and discovered that the reporting requirements are not consistently met. The audit could not definitively establish whether the reporting never occurred or occurred but was not documented in the patient’s health record. Answer #1

Every data reporting is legally bond . Any mal - presentation of data colud lead to legal actions because these data are headed forward to higher administration which makes the final output and publishes it . The legal issues which are present in this situation is that the data collected is not sure whether occured or not and also not sure about its documentation in the patients record . This could lead to discrepancy and faulty data reporting . The main legal action will be taken against the directors for presenting false data and they might lose their job , licence even fine and imprisonment .

To resolve this a team approch with good communication skills should be used . Democratic practice without any burden and stress will help to collect the data correctly and help to do the daily functioning efficiently .
