Pneumonia is a disease which affects the lungs and causes inflammation in the airways. This distrubs the breathing process. The patient care plan should include such as:

  • Nursing diagnosis: Inability of the patient to breathe properly.
  • Desired outcome: To clear airways passage for normal breathing, reduce the obstruction, decrease the pain. Elimation of the infection completely.

Nursing intervention & Rationale:

  • Assessment of the heart rate, respiratory rate of the patient. ( This will enables the nurse to intervene regarding the breathing problems.)
  • Assessment of the cough production. ( In case of pneumonia, the cough thickens.)
  • Vital signs such as blood pressure, body temperature etc. ( This will enables the nurse to understand the infection process)
  • Medication should be administered. ( Specially the use of antibiotics which eliminates the bacterial infection)
  • Encouraging the process of ambulation. ( This will help in mobilizing the secretions as well as decreases atelectasis.
