- assess for the client's posture and gait.

- assess for the patient's weight.

- assess for the range of motion in all the joints in the patient.

- assess for the patient's activity of daily living.

- assess the client's vital signs after the physical activity.

- enhance the client to increase the activity.

- assist the client to perform the range of motion activities and all her daily activities.

- encourage the client to take rest in between her activities.

- identify the required physical therapy from the therapist.

3. Risk for injury related to decreased bone functions.

Intervention:

- assist the client with range of motion exercises in both active and passive exercise.

- encourage the patient to loose weight in order to decrease the weight on the joints.

- educate the patient to use low beds inorder to possible fall from the bed.

- advice the patient regarding safety measures that are to be followed.
