  • The first step of the nursing care plan for the patient is to create a client data base.
  • The client data base includes the gathering information about the health of a client.
  • In this step the nurse identify the risk factor and diagnosis the client according to the risk factor.

2. PLANNING OF DIAGNOSES

  • Nursing diagnosis refers to the focusing, dealing and identifing the high risk problem of a specific client.
  • After that nurse plannig for diagnosis that client according to the risk factor.

3. SETTING PRIORITIES

  • Setting priorities is a process which refers to the establishing a sequence for nursing interventions.
  • It is the step in which the client and the nurse start planning which nursing diagnosis require first attention.
  • Diagnosis are gouped as high, medium, low level.In the high priority level Life threatning should be placed.

4. INITIATING CLIENT GOALS

  • In this step nurse initiate the goals for client for each determined priority.
  • Goals and outcomes refers to the nursing intervention which is derived from the client nursing diagnoses.
  • Goals gives the direction for evaluating client progress, enable the nurse to determine which problems have been resolved.
  • Goals have two type one is short term goal and anothe is long term goal.
  • Short term goal refers to ta statement distinguishing a shift in behaviour which cabn be completed immediately with in a few hours or days.In other hand the long term goal refers to the objective which is completed over a long period of time.

5. SELECTING NURSING INTERVENTIONS

  • Nursing intervention refers to the activities that a nurse perform to achieve client goals.
  • Intervention is used to reduce or eliminate the etiologyof the nursing diagnosis.
  • There are three types of nursing interventions these are, Dependent nursing interventions,independent nursing interventions,collaborative nursing interventions.

6. PROVIDE RATIONALE

  • Rationale refer to the scientific explanation underlying reason for nursing interventions.
  • Rationale doesnot appear in regular care plan of client.

7. EVALUATION

  • Evaluation refers to the activity in which the client progress towards the achievements of goals and the effectiveness of nursing care plan.
  • Evaluation is an important part of nursing care planning because the conclusion drawn from this evaluation step determine wheather the nursing intervention should be continued,terminated or changed.
