6. 99285 for emergency department visit of new patient. L23.7 for allergic reaction from poison ivy.

7. 99477 for initial hospital care of critically-ill neonate who is of 28 days or younger, and requires intensive care.

8. 99347 include home visit of established patient for E/M. 150.20 for congestive heart failure. 99495 for transitional care management (caregiver communicated through phone for complex medical situation).

9. 99367 for medical team conference of physicians related to different fields. It is done to review patient care. It does not include patient or patient’s family. It takes around 30 minutes.

10. 99201 for office visit.
