8. Patient's problem with self image and physique , lack of self esteem, and suicidal ideation is discussed with the primary care provider.Miss Mann's plan to reduce a weight of 10 pounds with in a couple of days is unrealistic, and potentially a dangerous idea as such there is a high risk for imbalanced nutrition, fluid and electrolytes imbalance , energy deficit if patient either choose to eat too little or eating and drinking not at all. So,Patiens diet shall be monitored. It must be ensured that patient eat proper and a balanced diet.
9. A)Miss Mann needs a proper counseling session in which a qualified health care professional shall check her BMI, and suggest a scientific, systematic and realistic plan to reduce her body weight if the need so is required. If there is a mismatch between her subjective perception and objective information she may recommended for further counseling.
B)As patient have expressed she is unwilling to live , Patient should be monitored continuously and observed to prevent any kind of suicidal attempts.It is advised that any object pointy or sharp must not be kept near by her surroundings.
C) Client's weight monitoring shall be done on daily basis and it must be ensured that she eats a well balanced diet. Patient's grievences should be addressed and her opinion should be sought while planning the nursing care. This is important as patient has expressed her unhappiness about thet given care / hospital setting as she is so happy to "leave the place".
