Risk for impaired skin integrity related to decreased circulation and sensation caused by peripheral neuropathy and arterial obstruction.

Desired outcomes

Patient’s skin on legs and feet remains intact while the patient is hospitalized.

Patient will demonstrate proper foot care.

Interventions:

  1. Assess the integrity of the skin .
  2. Assess knee ,deep tendon reflexes and proprioception.
  3. Use foot cradle on the bed
  4. Use space boots on ulcerated heels
  5. Use pressure relief mattress
  6. Wash the feet daily with mild soap and warm water. Check the water temperature before immersing the feet.
  7. Inspect the feet daily for erythema or trauma.
  8. Change stockings daily. Encourage patient to wear cotton socks.
  9. Use gentle moisturizers on the feet.
  10. Cut the toe nails straight across after softening the nails
  11. Never allow the patient to walk barefoot.
