active immunisation- immunisation is a process in which a foreign antigen is introduced into our body and the body's natural immune response is activated against that antigen and it is destroyed. A memory of this is being stored in memory B cells. when a real pathogenic organism having similar antigen enters our body, the body will produce a faster, stronger and more specific response to the pathogen and the pathogen will be destroyed before it causes significant illness.
passive immunisation- the antibodies which are prepared against an antigen is injected directly in a person who is suffering from or likely to suffer from an infectious illness. this will protect the individual from the infection or likely to reduce its complications.
so by taking immunisation, we will either prevent an illness or likely to reduce its severity and complications. infections like smallpox is eradicated because of effective vaccination. polio is on the verge of elimination because of vaccination campaigns.
