Active Learning Template Medication: Lanoxin.
Lanoxin
It is also called digoxin, it is the drug used to treat atrial fibrillation, atrial flutter, tachyarrythmias etc..
1.Pharmacological action.
Its action is to increase the cardiac output and it slowed down the rate of heart by showing positive inotropic effect,
Increases the contraction between the muscles of the heart, and reduces the conduction of arterial ventricular node and sinoatrial node.
2. Therapeutic use
A) used to treat arrhythmia and has a positive inotropic effect.
B) used in the treatment of congestive heart failure.
C) used to treat atrial flutter and atrial fibrillation.
3. Medication administration.
For immediate effect initially large dose is prescribed, and this drug should be given in various doses divided in 12 hr to 1 day.
In adults= up to 1mg is given at the beginning later it is given at the interval of 4 to 8 hours.
In children= up to 12 mcg /kg/weight is given at the beginning or for the initial dose later a time interval of 4 to 8 hours should be maintained between the doses.
4. Drug interaction.
It reacts with few diuretics such as loop diuretics and the thiazide.
Over the administration of laxatives should be avoided as it may lower the potassium level and causes toxicity with the Lanoxin.
It should be avoided with beta-blockers and thyroid drugs, as it may decrease the absorption of the drug.
5.Contraindication/ precautions of the drug Lanoxin.
it is not recommended in hypersensitivity, arterial ventricular blockage, alcohol abuse, etc.
Use with precaution in an imbalance in electrolytes, old age or geriatric clients, overweight client, a client who is pregnant or lactating.
6. Complications.
1. Weakness, headache, and fatigue can be observed in the central nervous system.
2. Unclear or blurred vision,
3. Heart= changes in ECG, low heart rate.
4. Diarrhoea, vomiting and nausea in the gastric system.
Blood/ hemat = Thrombocytopenia.
7.Evaluation of medication.
1.seriousness of the congestive heart failure is reduced.
2. Improved ECG reading.
3. Improves cardiac output.
4. The rhythm of the heart returns to the normal.
8. Client education.
tell the client to take medication as directed by the physician.
don't miss the dose.
take a dose at the same time.
the missed dose should be taken early as possible but within 12 hours.
check pulse before taking Lanoxin, contact physicians if >100 bpm and < 60 bpm.
don't put the medication in a container of other medication.
do not give your medication to others for consumption.
carry your identity badge written Lanoxin prescription.
9.NURSING INTERVENTION.
1. Check pulse apically and inform physicians for the dose.
2 check vitals especially blood pressure.
3. Check IV site for infiltration.
4. Electrocardiogram monitoring should be done by the nurse throughout the administration of Lanoxin.
5. Monitor urine output and the intake of fluids.
6. Check for overdose and toxicity if hypokalemia occurs administer potassium and if more complication arises inform to the physician immediately.
