SOLUTION:

As epinephrine is used to treat many conditions like anaphylaxis and cardiac arrest there is a dosage variation and different route of drug administration. whereas it's clearly known that the patient had anaphylaxis reaction and the route of administration should be IM. Crash cart in the EMERGENCY ROOM setting should be pre-loaded with the proper dose for each condition.

The error occurred may be due to

Lack of training knowledge or dosage calculation error would have occurred or wrong communication between doctor and nurse, The people in the emergency setting should be properly trained.
