A 3 year old girl presented with a low grade fever, listlessness, and a depressed appetite. Her pediatrician did a CBC and spinal tap to rule out meningitis. The lab findings were: WBC = 65.0 x 103/ul with 40% small blasts with small nucleoli. Abnormal blast cells were also noted in the CSF. Peroxidase stain was negative. Based on these findings and the patient age, the most likely diagnosis is

Answer: The findings shows that the WBC count was 65.0×103/up along with the small blasts with small nucleoli. The presence of abnormal blast cells noted in the CSF.

These results shows that the girl may have lymphoblastic leukemia. In this kind of case there is over production of the myeloblast where the cells leaks from the bone marrow and speeds up in the circulation.
