  • Is it true that you are veggie lover, vegan, or eat meat?
  • Rundown any sensitivities, including nourishment hypersensitivities.

Food/Drink Consumed

Did all of you expend a similar nourishment and drink?

  • Provided that this is true, what did the supper comprise of?
  • If not, what were the decisions?
  • Meat decisions:

  • Drink decisions:

On the off chance that water was offered, would it say it was packaged or tap?

Onset of Illness/Symptoms
