Electronic health records means documentation of the patient information in the form of controlled vocabulary rather than text.

The main data elements includes in the EHR list of problems of the clients, medications and it's allergies and patient related all information.

So here the data elements are

* screening for pain by using pain scales or visual analog scale, and record elements like type of pain, severity, frequency and site ect.

* screening for depression by using psychoanalytical scales, what are the causes for depression, level of depression, aggravating factors etc.

* Assessment of adverse health behaviors like smoking and alcoholism, body mass index more than 30, and use elements like what type of disease they are suffering like cardio vascular diseases, respiratory problems, kidney problems, liver problems etc.

* comprehensive or complete geriatric assessment the old age above 65 yrs, for more than 7 days hospitalization and readmission with in 3 days after discharge, the elements like all the systems functions, and complications due to hospital discharge processes etc.

* and to improve the care team performance the all health care activities and plans should be recorded clearly.

* By using electronic information exchange through transitions of care from admission to discharges, they will maintain the good communication between low level to high levels of health care facilities.

* through EHI electronic health information they will share among different health care professionals to provide good care and get good evaluation.

* third one is patient safety, by maintaining good confidentiality of the client information and maintaining security passwords we can prevent the theft of health information and and it like an alert.
