PATIENT REGISTRATION FORM

Five critical pieces of registration information:

1) Patient's personal details:

This includes the patient's name, address, location, date of birth, contact number. This will help to contact the patient in future for any purposes. This details will also helpful for the billing process.

2) Payment information:

This includes the patient's working details, location, type of work, office address, policy numbers, and details of the claim, the person responsible for payment.

3) Health insurance information:

The patient coverage and limit of claim, Whether eligible for insurance coverage, Having Medicaid or Medicare card, and whether receives state or federal assistance for coverage.

4) Medical History:

Patient's current medical status, allergic to any medications, location and time of injury in case of motor vehicle accidents and related data.

5) Special requirements:

Whether the patient required any special needs such as the need for an interpreter in case of the linguistic problem, stretcher or wheelchair for transportation or any other access required.

Notice of privacy practice:

Notice of privacy practices contains detail explanation of the patient's rights to the health information and HIPPA privacy rules on health plans and health care providers. It is the rights of the patient to get detailed information about the privacy of health information. It deals with the patient right to use and disclosure of patient health information. It concerns with health issues and concerns of protected health information. It consists of the general rule, covered entities, health plans, and organization options.
