* assess the urinary output ;to know intake output per day

*increase fluid intake through iv fluids as per doctor's order

*monitor vital signs

*monitor electrolytes:to assess excretion of electrolytes through urine

*administer bicarbonate if indicated

2.imbalanced nutritional status less than body requirements

*assess the BMI

* Food intake is inadequate due to high blood sugar value

*administer iv fluids.

*administer insulin as per order

* provide small & frequent diet

3.Risk for infection (sepsis )

* Assess the leukocyte count and other infection parameters like CRP, PCT

*assess the urinary output :if it is low also high risk for infection

*Check blood sugar minimum thrice daily :if it is high also chance to gets infection

*administer insulin as per order for better hyperglycemic control

*Administers antibiotics if indicated
