Informed consent

It is an legal requirement for medical and surgical intervention or invasive procedure involving patient.

Physician has the responsibility about all aspect of the treatment such as its risk, benefit, alternative and complication to patient. Failure to obtain informed consent renders any physician liable for negligence or battery and contribute medical malpractice.

Interpreter

Consent must be written in english and information must be verbally provided in narrative language. Interpreter may be staff, sometime patient refuses and choose friend or family member. Interpreter should sign as "witness " and specify their name, title, date and time.
