Process includes 6 steps

1. Assessment, For example vitalsigns, patient complaints, lab or radiology reports, physical assessment
2. Nursing diagnosis, For example imbalanced nutritional status, acute/chronic pain, ineffective breathing pattern
3. Goal : For example short term goals and long term goals
4. Planning : For example parameters monitoring, medication therapy
5. Evaluation: vital signs, physical assessment
