After you read this post can you help me write some response constructive, thoughtful and introduces new perspective.

Situation:M.C 90-year-old female who admitted due to fracture of left inferior ramus and sacrum zone by fall at her apartment a few weeks ago.

Background / Past Medical History (PMHx): Presented in ER CT scan of pelvis revealed superior and inferior left pubic ramus Fxs and left sacral alar Fx. M.C has been diagnosed with pulmonary fibrosis, pulmonary HTN, chronic respiratory failure with hypoxia for 20 years. Currently, fall at her home with immediate left hip pain. Allergic reaction to B12.Both parents have pulmonary fibrosis.

Physical Assessment Findings (head-to-toe):AOx4.BP 132/70mmHG.SPO2 98%, Sublingual Temp 97.8F, R 22 times/min. P 67 BPM. Complained moderate pain in the left hip. Regular heart sound and rhythm. Even radial and pedal pulses. Normal capillary refill. Pitting edema +1 that barely detectable impression. Regular respirations. Faint wheezing lung sounds but not crackling. Frequent nonproductive dry cough and difficulty to speak during coughing. SPO2 98 to 100 % during oxygen therapy. O2 at 5 L/min.Soft and nontender abdomen area. Ambulatory assistant required with a gait belt and W/C due to fracture of left side hip. Intact color and warm skin. No pressure ulcer site. Cooperative behavior.

Medication list:

  1. Tylenol(acetaminophen) 1000mg PO 3x/day
  2. Ultram (tramadol) 0.5-1 tb PO Q6H PRN
  3. Tessalon Perle, Zonatuss /benzonatate for the cough suppressant
  4. Sodium chloride nasal spray: 1 spray every hr PRN.
  5. Sildenafil 20mg PO a day PO for pulmonary hypertension

Analysis: The patient is administrated acetaminophen 1000mg PO 3x/day for mild pain and0.5-1 tb PO Q6H PRN due to her pain causing fracture of the left hip side. Because the patient has been experiencing pulmonary fibrosis and complications of its diseases. Her medications are that Sildenafil is Phosphodiesterase (PDE) Inhibitor impotence and pulmonary antihypertensice for erectile dysfunction and pulmonary arterial hypertension. sodium chloride nasal spray is Hypotonic crystalloid solution for add moisture inside the nose and relieve stuffiness and makes breathing easier. Benzonatate is antitussive and non-opioid for the relief of a nonproductive cough for the patient who has chronic respiratory failure. She complained hearing loss it may cause the side effect of Sildenafil.

Answer :

Yes, some times it may cause hearing loss, and it is a potential cause for hearing loss. The cause is still unknown.

* but some researches shown that in five cases of sudden hearing loss after taking this sildenafil medicine.

* and also some researches given report that two third of the peoples or clients loss their hearing capacity with in 24 hours after talking this sildenafil or Viagra .

* the action of this drug is vasodilatation due to increased cGMP , which increases the erection. The continuous erection also can damage the penis.

So may be due to dilatation of blood vessels to the sensory organ like ear, they may get hearing loss
