As the study of the case tells us that, Red Yoder is an 80-year-old farmer who lives alone in the farmhouse where he grew up. It is located 20 miles outside of town. His son, Jon, manages the farm now, but Red is still involved in the decision making. Red's current medical problems include insulin dependent diabetes complicated by an open foot wound. He also has some incontinence and difficulty in sleeping.

He relates that he has an open wound on his big toe that has developed after walking in a new pair of shoes.

During the nurse’s visit it was assessed that he has sleeping problems and some urgency incontinence, in addition is the breakdown on his toe. This need immediate care and medication, further assessments are needed of his diet, medication, and alcohol use, and to rule out elder abuse. For this he has to be hospitalized.

During his stay at the local hospital, he was diagonised for sepsis. The focus is an emphasis on the typical presentation of sepsis in the older adult. Further during the stay in the hospital some functional decline has occurred.

All these need a nursing care and personell attention for Red Yoder.
