1. This can be reported when there is the signs and symptoms of itching pain or swelling

2. It can be informed to the tissue viability nurse to monitor for the infection and skin. If needed the consulting physician may be informed.

3. The description for the documentation includes : a red discolouration present on inner thigh without any swelling or blister formation when measured it is moderate in size. The discolouration has not spread on any areas of thighs.

4. Medical treatment starts with the complete assessment of the skin site, the cause of the discolouration to be validated if there is any sign of skin infection it must be treated with appropriate monitoring of the antibiotics. Topical thrombophob if it's a blood clot

Nursing management includes:

* assessing the skin

* provide skin care

* educate the mother regarding the clothing

* massage to improve the blood circulation.

* application of cold or hot compress
