Documentation bu are working at a facility and performing per serve a moderate-sized red u ity and performing perineal care. Yo loderate-sized red discoloration located on your mgn. When will you report this finding? To whom wil kort this? How will you describe the discoloured areas?

Perineal care is one of the basic procedure. If a Certified Nurse Assistant is performing it.Any changes in the perineal area has to be reported immediately to the assigned Registered Nurse because it is one of the first sign of pressure ulcer.The staff should inform that the inner thigh is red in color ,which is moderately spread.The RN after inspecting has to record this in patient documentation and provide the necessary protocol as per the hospital policy.
