1.      What information would you want to find out about Mrs. Gregg and her home before beginning her care? (2 marks)

*Before beginning care for Mrs. Gregg I would like to know about the physical set up of her home, is she able to gain access to her washroom, bedroom and kitchen etc.

*Her likes and dislikes alongside her dietary practices

*What activities are involved in her daily routine if she has one

*Besides me being a companion for her does she have any friends or family who will be coming to spend time with her

 2.      List the types of information that you would expect to obtain from the care plan. (2 marks)

*What medications are being taken by Mrs. Gregg

 * Extent of her physical movement

*Has she been offered range of movement

*Does she have any special diet

 * Does she have any allergies

3. Situation 1 -

-Client’s Feelings

After a stroke patients may be experiencing difficulty swallowing, it may also be difficult for them when it comes down to coordinating certain movements (“difficulty getting food to her mouth”) as well as difficulty remembering stuff (“unable to choose the correct utensils “) & they  may also have a fear of eating because of their dysphagia which can cause choking.

-PSW Actions (physical and emotional help)

As a PSW you are here to support the client through this, therefore psychological support can be provided to the client in order to reduce her fears.

*She can be taught mild exercises as well as exercises which include range of motion in order to help with her coordination.

*Help Mrs. Gregg regain her memory be having her repeat things, practice her reading, have her meditate as well as work on her concentration

*Reorganize utensils or find a way to help her grip her utensils

                       Situation 2 -

                      -Client’s Feelings

Mrs. Gregg is experiencing loneliness & has memory loss to a certain extent where it’s been difficult to remember some words and on top of this she is also in a state of boredom therefore she wants to try her best to recover in order that she may return to work.

-PSW Actions (physical and emotional help)

Spend extra time with her, find different activities which can encourage her reading skills with hope of her regaining her memory little by little such as games, listening to music as well as finding other ways to help her express herself better so that she won’t feel so down or lonely/ bored. Improvements in her mood can also help with her recovery process.

Situation 3 -

-Client’s Feelings

The client is going through a state of depression because of what has happened to her and how her life is completely different. Depression causes a through cognitive changes. They show loss of interest and are in a constant feeling of sadness.

-PSW Actions (physical and emotional help)

The main focus of the PSW in this situation should be the cause of Mrs. Gregg’s depression and her change in attitudes and behaviors. The PSW should be empathetic towards the client and show that they’re willing to listen to listen to her feelings and to help try to change her mood. Try as much whenever it’s possible to help her with her self-dependency, because the fact that she is having to depend on someone else to help her go about her day is also contributing to her depression.

Situation 4 -

-Client’s Feelings

This is all new to Mrs. Gregg, she is in an uncomfortable situation so therefore she doesn’t want anyone to attend to her in the bathroom. She’ll try in every possible instance to be independent.

-PSW Actions (physical and emotional help)

As a PSW one of our duties includes promoting independence of the client whilst assisting the clients with their daily routine activities. We also provide help and comfort to the client in order to reduce the feeling of loneliness and respect the client’s wishes. Therefore in this situation the PSW should respect Mrs. Gregg’s wishes to move independently and should only intervene if a situation occurs where she requires assistance.

4.      Using all of the above information, describe how the following factors might affect Mrs. Gregg’s relearning of speech and mobility. Use case study information to help answer questions. ( 3 marks)

a.      Age

The age factor affects Mrs. Greggs relearning of speech and mobility because she’s a 55 year old woman and as we human beings age our brains and nervous systems go through natural changes. Our brains and spinal cords lose nerve cells and weight which leads the nerve cells to begin passing messages slower than they did in the past.

b.      Individual characteristics: personality

This is based upon the strong will power of Mrs. Gregg, strong personality helps with the quick relearning of speech and mobility.

 c.       Environment

Mrs. Greggs social as well as physical environment have an effect on the relearning of her speech and mobility, if the environment in which she’s in is a supportive one then that favors her relearning process.

5.      Describe five (5) ways in which the PSW can promote Mrs. Gregg’s self-esteem and motivation during her daily routines. (5 marks)

a.      Try as much as possible to make her experience fun

b.      Compliment her when she’s done something

c.       Promote independence

d.      Provide psychological & emotional support

e.      Try to have her engage socially

6.      Define Restorative Care and what role it would play in helping with Mrs. Greg’s rehabilitation. How could the PSW assist with this process? (2 marks)

Restorative care is a type of care which helps a person to regain their health, strength and independence which includes follow up care and rehabilitation. This care plays a significant role in helping with Mrs. Gregg’s rehab. It helps to preserve her independence and optimum functioning level. It can slow down or either reverse her difficulties when it comes to her daily activities as well as help her to adapt to her changing needs.

As a PSW we could assist with process by motivating Mrs. Gregg, helping her to carry out daily activities as well as promoting her independence.
