1.Some of the information which can be gathered before initiating the care are

• Identifying the dietary patterns to ensure the right type, quantity, preferred choices of foods ,dislikes to plan for care
• Assessing for any other medical illness,medication etc., to be covered
• Knowing the patients free time activities ,daily routine to plan care
• Cultural assessment to provide a cultural competent care.
• The patient has the habit of practicing regular exercises daily

Situation 1

Client's feeling

The patient after a stroke may have difficulty to swallow foods,they may have difficulty to exhibit coordination of movements(difficulties eating) ,memory impairment, difficulty to remember things(problems with selecting the right utensils) ,they may have a fear of eating because of their dysphagia which can lead to choking (threw the plate)

The patient has to be taught about mild exercises,range of motion exercise to ensure coordination

Visual scanning techniques can be used

Repeating things, meditation, concentration,reading practice can help her to regain the forgetting things

Provide psychological support to recice fear

Situation 2

The patient is in a state of boredom and loss of memory to an extent which makes her difficult to regain the words and to quit her loneliness, boredom she wants to resume to work again.

Provide diversional therapy to quite loneliness
