Mrs. Gregg is a 55-year-old widow who has had an active career as a lawyer. She is an avid reader. She is an independent woman who has had to adapt to many changes in her life. She enjoys crafts and sings in the choir at her church. She has an adult son and daughter, and two grandchildren who visit her frequently, she lives in her own home, a small bungalow in the city. 


You are assigned to give care and support to Mrs. Grers, who had a stroke two weeks ago. She is paralyzed on the left side and has difficulty speaking. She is able to hear well and can understand when you speak to her. The long-term goals for her care include:

  • regaining mobility 

  • regaining speech 

  • being as independent as possible 

  • preventing complications such as contractures and pressure sores 


You are required to assist her to participate fully in all activities of daily living, and assist to meet her physical, safety, and psychosocial needs.

1. Mrs.Gregg certainly needs to regain mobility and speech and should be as independent as possible. The caregiver should prevent any complications like pressure sores and contractures. The caregiver is also required to assist her in participating fully in activities of daily living and assist her to meet her physical, safety, and psychological needs. To fulfill all these, the caregiver should know the safety measures in the home and the functions/movements that Mrs. Gregg is capable of doing herself. She should also know the past and present illnesses, medications, allergies, and dietary restrictions of Mrs. Gregg. She should also be aware of the details of the family members of Mrs. Gregg. Knowing the habits, likes, and dislikes of Mrs. Gregg is also important.

2. Types of information expected to obtain from the care plan are:

  • Assessment
  • Nursing diagnosis
  • Inference
  • Planning
  • Intervention
  • Rationale
  • Evaluation

3. The feeling expressed by the client will be that of helplessness. She was an active lawyer and an avid reader, who adapted to many changes in her life. Being bedridden is something unthinkable for a person of her caliber and helplessness will be the only feeling left with her. The personal support provider should boost her morale by supporting her psychologically. She should provide comfort and care by reaching out to the client at regular intervals and helping her to become active.
