Ordered dose - Lidocaine titrated from 2 to 4 mg/min for the patient with ventricular arrhythmias.

Pharmacy send 1 gm Lidocaine in 500 ml D5W.

You need to give the lowest dose of Lidocaine(2mg/min).

So, you need 120mg per hour. 2mg/min x 60min = 120mg/hr

1 gm(1000mg) Lidocaine in 500 ml D5W.

So, 1mg Lidocaine in 0.5ml. 500ml = 1000mg = 0.5ml/mg

So, 120 mg will be on 60 ml. 0.5ml/mg x 120mg = 60ml

Answer - The nurse is correct to start 60ml/hr for the lowest dose.
