Heart failure is a potentially disabling condition requiring significant knowledge to manage requirements of self care .the need for self care is important for all patients who are living in rural areas that are geographically remote from health care services

Rural people are having poor health care services , eventhough they have the facility of telemonitoring benefits are very less because they are the illiterate people who doen't understand the doctors orders from the telemedicine .

Other biggest issue is the electricity problems which influence to poor telecommunication facility .

The people with CHF should take care them selves such as taking care of diet ,lifestyle modification ,exercises and regular followups about their health status and taking of regular medications

If the people dont take precautions about their health conditions they may prone for risk and they need to readmitt in the hospital.

As a health care providers We need to educate the patients about home care management of CHF .

By conducting health awareness programmes .

Advice the patient for regular health checkups and followup care .

Educate the patient about their lifestyle modification ,about their diet what type of diet they need to take such as fat free diet etc .
