Robert T. is a community-oriented nurse and a member of the Jonesville Healthy City Committee. Also on the committee are the mayor, a hospital administrator, several ministers, an epidemiologist, and several other community leaders. Through an initial community assessment, the Jonesville Healthy City Committee identified several areas of need. These areas include lack of health care access for the homeless and uninsured, lack of knowledge about health-promotion behaviors, and substance abuse among teenagers. 


The committee shares its assessment results with the community at a town meeting. Through the group discussion that follows the committee decides to focus first on addressing the need for improved health care access for the homeless and uninsured Many members of the community state that they would like to see a homeless shelter built that would provide lodging and free health care services. Nurse Robert suggests that a nurse- managed clinic would provide a possible way to improve health care access for the uninsured. The committee continues to discuss possible solutions to the health care access problem and ways to obtain funding for the projects previously discussed.

The two most appropriate and widley used promotion approaches are top down and bottoms up approach. Here, the committee has used the top down approach.

The bottoms up approach is referred to as the approach thate peices the system to give rise to more complex system. The data here is gathered from the employees or the lowest hierarchical level of the system, the advice or the feedback from client in health promotion may be the initiation of this type of system.

Top down approach is the process where the system is broken down and designed to gain in-sight to sub-system, the program in health promotion that first specifies the disease and need and then produces a plan for focus in community is top down approach as it is used in this case.
