ordered dose=7.5 mg/kg q12 h

strenghth of drug=250 mg/ 5 ml

weight=154 lbs=69.853 kg=70 kg (approximately) ( as 1 lbs =0.453 kg)

required dose by the patient=weight * ordered dose

=70 * 7.5 =525 mg

If 250 mg in 5 ml

then 525 mg in X ml

  \therefore 250 * X=5 * 525

\therefore X=5 *525/250=10.5 ml

Therefore patient should be administered 10.5 ml of antibiotic dose.
