The change is relating to redesigning system processes, policies, and structures to provide opportunities for and support of relationships between patients, families, and the health care team is important to facilitate patient engagement.

Answer: To facilitates the patient's engagement it is important to includes the health status issues and probly faced by the people. For changing policies it is important to understand the need of the patients. The policies should be budget friendly so that it can be easily followed. The implementation of the changes in programs requires specific concern. Making changes in purpose, goals as well as establishing the feedback process from the patients to improve the particular drawbacks.
