1.Altered body temperature related to infection as evidenced by increased neutrophil count secondary to chemotherapy

Intervention

  • Monitor vital signs as it is the baseline to plan for care
  • Administer antipyretics to reduce the fever
  • Follow aseptic techniques ,hand hygiene infection control practice to reduce the rate of infection
  • Provide tapid or cold sponge to cool the body

2.Risk for poor surgical outcome related to infection

Intervention

  • Administer as per order to control infection
  • Treat the underlying cause

Risk for fluid volume deficiency related to chemotherapy secondary to wilms tumor

Intervention

  • Monitor intake and output chart to plan for care
  • Administer IV fluid to maintain fluid balance

Dose:15mg/kg

The child weighs 12.7 kg

When 1kg=15mg

Then 12.7kg=??mg

Cross multiply and divide

=12.7×15÷1

=190.5mg

3.Available: 5ml=160mg

Therefore 190mg=??ml

Cross multiply and divide

=190×5÷160

=5.9ml

Ans:Draw 5.9ml

4.Ibuprofen has a nephrotic property si it is not recommended for a child with wilm's tumor

4.weight:92 pounds(41.7kg or 42 kg)

Order:0.1mg/kg

Therefore 42kg=??

Cross multiply and divide

=42×0.1

=4.2 mg

6 .Available 2mg=1ml

Therefore 4.2mg=??ml

=4.2×1÷2

=2.1ml
