Module 06 Classroom Assignment - Assess and Document

Prior to class, and after reading/viewing the module assignment, select one of the following and conduct an assessment.

You may conduct the assessment on a fellow student, friend, or family member. Remember to secure their permission.

Collect both subjective and objective data using the process described in the textbook.

Then, document your findings and bring them to class.

Assessment of head: ( skull, scalp, hair) : first the size and shape and contour of the skull and then scalp is observed in severalareas by separating the hair at various locations inquire about anu injuries,(presences of lice , nites, dandruff or lesions) palpate the head by running the pads of the fingers over the entire surface of the skull, inquire about tenderness upon doing so ( wear gloves if necessary) and observe and feel the hair condition, normal finding: skull : generally round with promienences in the frontal and occiptal area and no tenderness noted upon palpation, b) scalp:generally lighter in color than the complexion and can be moist or oily and no scars noted and free from lice and nits and dandruff and no lesions should be noted and no tenderness or masses on palpation, c) hair : it can be black brown or burgundydepending on the race and evenly distrubeted covers the whole scalp and may be thick or thin coarse or smooth and neither bright nor dry.
