1. A 4 month old with a history of hydrocephalus has undergone surgery for placement of a VP shunt and is ready for discharge.

1)1.Hydrocephalus term itself refers to hydro =water and cephalus= brain.Therefore it is the accumulation of fluid or water in the cavity of the brain.This cavity ha termed to be ventricle.

2.The following things has to be monitored for

  • Assess the neurological status of the child for consciousness, irritability
  • Bulged anterior fontanelles
  • Enlarged head
  • The cry of the child which are generally high pitched
  • Sucking reflex can be checked
  • Assess for any fever,vomiting
  • Monitor ICP

3.The following information has to be included in the teaching plan

  • Report on case of new signs and symptoms in the infant
  • Keep the surgical site dry and clean
  • Regular follow ups

2) 1.The different kinds of seizure are

  • Grand mal seizure where patients loss their consciousness and develop muscle rigidity
  • Tonic seizure refers to muscle stiffness ,loss of breathing
  • Clonic seizure refers to involuntary movements of the entire body parts
  • Myoclonic seizure are intermittent type of seizures
  • Atomic seizure the sensation is lost in the limbs
  • Absence seizure refers to loss of consciousness for few seconds and can occur throughout a day en number of times with no patient self realization

2.The following information has to be asked

  • Any facial droppings, loss of limb function, and difficulty of the child to converse
  • How long the seizure lasted
  • Any is the child has any history or family history
  • Assess for risk factors

3,The patient can be protected in the following way

  • Make sure no sharps ,hard substances is bear to the patient
  • Make the patient lie in recovery position to prevent aspiration due to secretion
  • Administer medication as per order

4.Administer anti convulsant drug to stop seizure and sedate the patient to relax during an ongoing seizure.
