1. A 4 month old with a history of hydrocephalus has undergone surgery for placement of a VP shunt and is ready for discharge.

1 . It is one of the medical condition seen on the newborn, where as the head of the baby is quite large comparing to the normal size because of the presence of water like fluids get stored in the baby's head region within the internal organ's cavity.

2. The baby with this specific condition, after doing this surgical procedure continuously monitor following things such as

• check the Intra cranial pressure closely
• Check for any sign of diarrhoea
• find any bleeding or clot present over operated region.
• Monitor the 4 month baby for any kind of infection over the operated region.

3. The things that are need to instructed to the mother with this kind of baby are as follows,

• Explain the type of position that need to maintained by the baby
• Explain the medical and drug follow up procedure to the mother.
• Clearly explain the notable complication that can be occurs after having this surgical procedure,
• Explain the mother about the fluid intakes and restriction to the baby.
• Advice the mother about the coping mechanism and recovery process for these condition.
