1.a 14 year old is having diagnosed with systematic lupus Erythematosus.he is taking corticosteroid long term.

A).for discharge plan Patient with his family members should be included beacause SLE is a autoimmune disease where the immune system starts attacking own body rather than harmful materials.in this disease there will be two phases one is active phase where the patient can have some symptoms and other is remission phase where the patient will be asymptomatic.so for discharge plan patient and his family members should be informed about all the possible happenings.

B).in discharge planning patients family members should be properly instructed about the following medication:

Steroids for prevention of infection.

NSAIDs for pain relief.

Immunosuppressive for slowing down immune system.

Cytotoxic for prevention of inflammation.etc

.patient should take rest when be will feel tired.

.his skin should be avoided from sunlight exposure beacause that can cause more rashes on his body..

.if the child has any kind of allergies or any adverse effects he should be taken to hospital immediately.

C).for assessment of SLE child skin can be assessed for lesion or rashes. It has to determined that where the child is standing now,if he is in active phase or in remission phase.if the child is passing through active phase then his parents should be very careful about child's daily activities.
