The Purnell Model for Cultural Competence by Larry D. Purnell and Betty J. Paulanka was formulated to develop intercultural competence among nurses and provide a skillful and quality care in their patient service. It has a brought a dramatic change with innovation in primary, secondary and tertiary care when a nurse has knowledge on transcultural or intercultural concepts. Cultural competency develops step by step in a person from completely not knowing about a culture, then learning it and finally implementing it with greater competency.
This model involves the person,environment, health and how the nurses encounters and handles them effectively called metaparadigm.It is mentioned in the outer circle of the model.
The inner circle of the model has twelve domains in it namely
●communication(language barriers has to be addressed ●spirituality (this differs greatly from person to person and so on and has to be handled diligently otherwise this can land up in many controversy ●Death rituals(mourning differs according to culture like some may wera white or black colored clothes as morning,burial or burning depends upon their practices and beliefs ●Pregnancy,Child birth and rearing(the foods,lifestyle ,activity, during and after child birth are greatly influenced by culture ●Healthcare practices(the type of care like allopathic, natural, herbal,chiropractice ,complementary medicine ,organ donation etc.,●Rules of the family members ●Behaviour in the family which may be violence and non violent behavior,alcohol, substance abuse ,etc.,●heritages●biocultural practices(difference between race and culture)●issues in working environments(work pressure,timing ,type of works,workplace harrassment, eyt) ●the healthcare professionals ●nutrition (certain culture restrict certain foods ,have greater concern on what they can eat and not ,time frames fir eating before sun set)
Hence this model can be used in practices, learning ,in administration and on research purposes in nursing and other fields.
