#. To diagnose subcutaneous fat loss assess for the following :-

-observation of areas where adipose tissue is normally present

-loss of fullness or areas with loose or hanging skin

-hollowing of orbital fat pads

To diagnose muscle loss assess the following areas :-

- temples

- shoulders

- clavicles

- scapular region

- hands

- quadriceps

#. Interventions made to help Hilda reach a goal of consuming >75% of meals are :-

Physician - to plan what type of diet she should have in consultation with the dietician according to her disease condition .

Nurse - implementation of the diet plan , providing emotional and psychological support, helping her to adhere to the diet plan

Dietician - planning the diet for hilda, making a diet according to the likes , cultures etc of Hilda.
