* one of the major reason is the mistakes in the conveying of information or commonly we can say as communication issue.This issues mainly occur among the doctors,nurses or clients.
* The next important reason is human issues and this can happen at the time when the quality of care,strategies or plan of actions are not obeyed systematically and clearly.
* Another one is the technical problems and this can involve the difficulties and lack of success by medical equipment or tools.
