Did you know that as of 2014, an estimated 400,000 people die each year due to medical errors? Why do you think this is? List several ways that you can prevent medical errors within the workplace.

In medical field of diagnosis and treatments, there are both aspects working side by side and they are: making the patient live or die. There are many reasons that happen in the hospitals leading to fatal results. Such practices should be taken care off like:

a; The diagnosis should be made properly with thorough basic check ups. The initial records should be made properly and if there is any inaccurate or improper information, it should be highlighted during care plan. Proper documentation should be done with accurate entries about the patient.

b; Medication should be proper and listed with clear documentations along with the support of client's family so as to keep everything in order and to avoid any bad happening.

c; Double or triple check the procedure of treatment plans. Counter check them with other staff nurses or doctors so as to avoid any discrepancy later.

d; Planning out the treatment with efficient and clean documents of care plan that what medicine to be given in how much dose at clear time. A proper charted plan should be made and kept near the patient so as to avoid any tension scenes.

e; Monitoring the patient at proper time intervals while keeping in mind about the risks involved during the treatment and being ready for any time high alert or emergency situations, so as to decrease the fatal probability.

Such documented and clear charted plans with efficient techniques and proper medications will along with the full consent of the doctors and the family will definitely end up in positive results for the patient. Such practices will never lead to fatal results.
