The reasons why most businesses operate at below capacity includes :

Lower than Expected Demand:Lower demand would make the company to underutilize the capacity

Seasonal Variation in demand : Seasonal Variation would make the company to underutilize the capacity at certain time of the year

Maintenance and repair : if there is maintenance and repair is scheduled it would make the company to utilize only part of the capacity

Loss of Market Share : In case if the company looses its market share it would force the company to produce less of the product which in turn makes under utilize  the capacity

But not Human Error : since it would not hit the capacity but it would hit the quality of the production
