You are a nurse working on a busy medical-surgical unit and have a 5-patient assignment today. You receive handoff report and make a mental list of all the things you need to do, which includes:

Take initial vitals signs on a newly admitted patient.

Perform physical assessment on your patients.

Reassess vital signs on a patient who was given a new cardiovascular medication.

Create a care plan for a newly admitted patient.

Document routine vital signs.

Check a finger stick blood glucose.

Evaluate if a dose of pain medication was effective.

Feed a patient.

Empty a urinal and record the output.

Weigh a patient.

You realize you will need help from your team. You prepare to make a plan with the unlicensed assistive personnel (UAP) who is assigned to your patients.

Ans) The Five Rights of Delegation are:

1) Right task:

Appropriate activities delegated must fall within the respective scope of practice.

2) Right circumstances:

Appropriate patient setting, available resources, and other relevant factors considered.

3) Right person:

Competencies must be established for the person who will carry out the task.

4) Right direction and communication:

Clear, concise description of the task, including its objective, limits, and expectations.

5) Right supervision:

Appropriate monitoring, evaluation, intervention as needed, and feedback.

Delegation of tasks to UAP:

- Smple, routine tasks such as making unoccupied beds, supervising patient ambulation, assisting with hygiene, and feeding meals can be delegated.

In the above listed tasks we can delegate UAP to:

- Feed a patient

- Empty a urinal & inform nurse to document it

- Weigh a patient

In order to determine if a task is appropriate to delegate or not, the nurse must consider:
• If there are laws & rules in place that support the delegation
• If the patient has been assessed
• If the task is appropriate for the delegatee
- Is the task within the UAP's scope of
practice?
- Does the UAP have the knowledge & skill to
perform the task?
• If the agency has policies or procedures in place for this task that must be followed
• If the proper supervision is available
