During a review of competition video, you observe that Mr Q is unable to maintain the orientation of his trunk during the later stages of his run (excessive extension in his lumbar spine and excessive lateral tilting of his pelvis). It appears that he is unable to use his trunk and hip abductor muscles to prevent that from happening. You decide to assess the control of Mr Q’s trunk muscles.

a) List one (1) test that you would perform to evaluate his ability to control his trunk muscles and justify why you have chosen it. (4 marks)

b) Briefly describe one (1) exercise that you prescribe to Mr Q to improve his hip abductor endurance and one (1) progression of this exercise. (4 marks)

You observe that in addition to the previously mentioned strength and endurance issues, he appears to have some shortening of his hip flexor and hamstring muscles.

  1. List and describe one (1) test you would perform to assess the length of his iliopsoas and one (1) to assess the length of the hamstring muscle group. Describe specifically how you will ensure the repeatability of each test and how you would quantify extent of differences in length between sides.
  2. Briefly describe the home programme (one exercise) that you provide to Mr Q to improve the length of his tensor fascia latae. List all exercise variables (sets, repetitions, frequency, and duration).

a)Here Mr Q has noted with excessive extension of his lumbar spine and tilting of his pelvis . So inorder to rule out the problem of Mr Q we can do a Trunk Contol Test which is done to assess impairment of motor functions.This test consists of 4 movements ;

1) Ask to roll from supine position to weak side

2) Ask him to roll to the strong side

3) sitting up from lying down position

4) Assessment of sitting balance for approximately 30 second

The scoring is from 0 to 25 .

0 is given when he is unable to complete the task without assistance

12 is given when he perform the activity in an abnormal way

25 is given when the task is completed withou any assistance

The maximum score is 100 and minimum score is 0 .

b) Total 6 muscles collaborated to perform hip abduction . Lie down on the floor with legs bent . Lift one foot off the floor . Lift hip up in the air , contracting your gluteal muscle . This excercise will improve his condition with flexible and strong hip .

ILIOPSOAS;

ileopsoas is a group of hip flexors in the abdomen and indicate that inflamed appendix. To assess the length of iliopsoas The Thomas Test is done .

Lie supine and flex both knees using both arms to ensure that lumbar spine is flexed and flat on table and avoid posterior tilt of pelvis . The test is positive when ;

patient is unable to maintain their lower back and sacrum against table

hip has large posterior tilt

knee unable to meet more than 80 degree flexion

HAMSTRING MUSCLE;

To assess the lengths of hamstring muscle the accurate test is Mannual Muscle Testing .Here the individual will lying on prone position with their knee partially flexed and their thigh medically rotated . Hold your leg in place while your partner tries to push the leg down .

HOME PROGRAM TO STRENGTHEN TENSOR FASCIA LATAE;

1) Lie on one side by supporting head by bending your arms

2) Bend leg to the floor for stability

3)straighten your upper leg , flexing your foot upward 90 degree

4) raise your leg at 50 to 70 degree angle

5) return to starting position

6)keep your leg straight and foot flexed throughout exercise

7)keep in mind your foot should not touch floor .

Mr Q can continue this exercise daily to improve his muscles .
