Answer #1

1. ORDER : AZACTAM 500 mg FREQUENCY : EVERY 12 hours DOSE AVAILABLE : 1 gram

a) ANSWER : 5ML . The patient is getting 1gram of Azactam per day ( 500mg every 12 hours). Remove 5 ml ( 500mg) from the vial after reconstitution with 10 ml of sterile water from the vial.

b) To calculate the rate using hours, the formula is VOLUME \div HOURS = ml/hour

Volume = 100 ml, HOURS = 1 hour (60 minutes)

100 \div 60 minutes = 1.7 ml/minutes.

c) To calculate the drop factor using 20 gtt/ml, the formula is

VOLUME (ml) \div TIME ( in minutes) x DROP FACTOR (gtt/ml) = 100 / 60 x 20 = 33 drops/minute.

2. Order medication is Cefazolin 6.25 mg/kg, Frequency - 6 hours (4 doses a day), Weight of the child = 38.2kg

a) ANSWER L: 3.75 ml

After diluting the medication with 10ml of sterile water, 3.75ml should be removed from the diluted medications because the child is getting only 6.25mg a dose.

b) To calculate the milliliters per hour, the formula is

VOLUME \div MINUTES = ml/ hour

Volume = 50ml, Minutes = 30 minutes = 50\div30 = 1.7 ml/minutes

c) To calculate the drop per minute, the formula is

VOLUME \div TIME (in minutes) x drop factor

Volume = 50ml Time = 30 minutes Drop factor = 20 gtt/ml

50\div30 x 20 = 33 drops/ minute
