  • Interoperability   In 2011, CMS established the Medicare and Medicaid EHR Incentive Programs (now known as the Promoting Interoperability programs) to encourage clinicians, eligible hospitals, and CAHs (Critical access Hospitals) to adopt, implement, upgrade (AIU), and demonstrate meaningful use of CEHRT (certified electronic health record).Beginning in 2019, eligible hospitals and critical access hospitals (CAHs) are required to report certain measures from the Promoting Interoperability Program’s four objectives, with performance-based scoring occurring at the individual measure-level. The scores for each of the individual measures are added together to calculate the total Promoting Interoperability score Sof up to 100 possible points for each eligible hospital or CAH. Score ≥50 points will satisfy the requirement to report on the objectives and measures of meaningful use, which is one of the requirements for an eligible hospital or CAH to be considered a meaningful EHR user and avoid a downward payment adjustment. Eligible hospitals or CAHs scoring < 50 points will not be considered meaningful EHR users.
  • So to stay with avoidance of downward payment adjustment of CMS service this point should be highlighted and analysed
  • Patient safety……….. A poor EHR design and its implementation as well as lack of proper training have impact on safety of the patient.

  • Usability by testing the EHR system specifically for safety or develop plans to detect risk within these functions For example, a clinician may inadvertently prescribe the wrong dose of a drug or order the incorrect medication for a patient which is an ongoing issue that has resulted in clinician burnout and patient harm.So proper designing, training and most importantly sincere efforts are the keyword in improving patient safety in EHR.

  • Patients privacy and security

The barrier to entry of adoption of electronic health records in the healthcare industry is privacy of patients and the security of their information . Legal regulations will be the most important to analyze and discuss prominent security techniques for healthcare organizations . To penetrate that legal barrier while seeking to get data from a secure electronic health records system is difficult.

Health Insurance Portability and Accountability Act (HIPAA) tells us about confidentiality and security of protected health information (PHI) in a patient’s electronic health record.HIPPA was passed by Congress in 1996. The three pillars to secure protected health information as sketched by HIPAA are administrative safeguards, physical safeguards and technical safeguards . These three tenets are also known as the three security safeguard themes for healthcare. These themes range from different techniques starting from location of computers to the usage of firewall software to protect health information. A brief list of safeguards and their definitions is provided below

A.Access control (technical safeguard) is a technique that prevents or limits access to an electronic resource, it limit access to only authorized parties. The healthcare facility collects, stores, and secures patients’ data, which is very sensitive.

B Physical access control (physical safeguard limits physical access to resources. It limits access to only authorized parties. A surgical patient in a facility will not have access to any clinic or ward except the surgical facility. A clerk in the diabetic clinic will not need access to the emergency room, so his/her access card will not open those doors.

C.To improve the patients security and privacy, policies, practices, and procedures in the facility to regularly check for vulnerabilities is the administrative safeguards.

So these above points should be discussed and analysed while sorting out difficulties in that particular hospital.
