Population health is defined as the health outcomes of a group of individuals, including the distribution of such outcomes within the group.(Stoddart,2003)

Healthcare delivery system is delivering health care services towards disease prevention and health promotion by organisation of people, instituitions, and other resources.Two main objectives of healthcare delivery systems are namely are quality cost effective care and right care at right time.

The ways by which a population health focus can be transforming health care delivery system are:

1. Accurate data collection will be made as the organizations need to be tracking patients throughout course of care.
2. Care coordination will be enhanced to promote population health which in turn transforms the healthcare delivery system.
3. Revenue generation and increasing market share
4. Information transparency
5. Patient engagement :When patient necessity is addressed and met , it stengthens the healthcare delivery system.
6. To promote optimal health in population continued care should be delivered , So multidisciplinary care is essential .
