Population health model is defined as the distribution of the health outcomes for a particular group. This is the new approach to health care that aims at enhancing and providing better health to entier human population in the world.

The focus of population health model is to increase the life expectancy and the quality of life of people living with this increased life expectancy. It also has aim to establish better equal health care with reduced health disparities.

Population health model is based on concept of determinants of health, the right side of model describes various health determinants that cause biological, social, physical and mental health issues. The model also includes policies and procedures to explain its influence on population health and hence, rhis is a broad model that includes almost everything.
